We recommend that you contact us in advance of your first visit for specific information about your policy, so that we can all be aware of your insurance status before evaluations and treatments begin. In order to make your first visit as productive as possible, we also encourage you to visit our First Time Patients section of the website, so you can fill out forms in advance.

First Time Patients

If you haven’t had your hearing tested in a while, we encourage you to schedule an appointment. Over time, our ability to hear can degrade, particularly among men. Early hearing loss detection and treatment can make a major difference in quality of life.

If you are experiencing symptoms such as difficulty hearing certain sounds or conversations, tinnitus, also known as ringing in the ears, dizziness, or balance issues, please plan to schedule an evaluation at your earliest opportunity.

If you’d like, you can fax the completed forms to us at 919-762-0359 before you arrive. Or simply bring them along to your first appointment.

If you have Medicare, or if your primary insurance plan requires a formal referral before any examination, treatment, or service is provided. Please copy this Medical Referral Form and have your primary care physician fill it out before your appointment.

If you don’t see the name of your insurance provider listed below, please be sure to let us know. We are always adding more providers to our service list.

All co-payments are due at the time of your visit. As a courtesy to our patients, we are happy to file your insurance claims for you.
